The lively voice on the line guided us in classic Latin American style: "Pass the flower market," she said, as we crept along a road flanked by stalls brimming with roses, daisies, and sunflowers. After turning right, then left, then left once more at the new concrete wall, we would spot the dirt path. "The home at its end - that's ours."

We reached Violeta Chuc Sam's family residence in the highland town of Quetzaltenango, Guatemala, on a windy Saturday morning in February. Chuc Sam, her energetic demeanor echoing the phone voice, welcomed our group of reporters at the entrance. Weeks prior, she had invited us to learn about her mother, Francisca Violeta Sam Colop. Our focus was Sam Colop's prolonged battle with skin cancer and her quest for Keytruda.

This fieldwork formed part of the Cancer Calculus probe by the International Consortium of Investigative Journalists and 47 media outlets, including my companions - reporter Jody García and photographer Laura Garcia from Guatemala's independent Plaza Pública. Our group devoted months to probing the worldwide fallout from Keytruda's steep prices, an innovative immunotherapy called pembrolizumab generically. As the planet's top-selling medicine, it has generated roughly $163 billion for Merck & Co. since debuting in the U.S. in 2014.

I first spotted Sam Colop's name last August in a 2021 court ruling that approved her Keytruda access, unaffordable otherwise. By discovery, I had scanned many parallel judicial battles and uncovered scores more later. Yet hers lingered. Diagnosed with advanced melanoma in 2012 - the condition Keytruda treated to achieve remission for ex-U.S. President Jimmy Carter - she was an Indigenous Maya K'iche' teacher, poet, researcher, and champion of her community's language and heritage. The prospect of humanizing her tale beyond court papers thrilled me. Then another search hit: a short obituary from July 5, 2025. She featured in records, but her voice was silenced.

Months on, I sat in her parlor with daughters Violeta and Marina, plus widower Juan Chuc Xum, under a big portrait of the resolute Sam Colop as they shared memories. Some of her works - a K'iche'-Spanish children's Popol Wuj (Maya sacred scripture) and her final poetry volume - sat tidily on a table. They recounted her life in turns. At 65 when she passed, Marina quipped she stayed youthful in spirit. A reader, singer, dancer with endless ventures, Violeta noted. "She always liked looking sharp," Chuc Xum added, using present tense for his wife. In 2021, an oncologist beyond her public insurance prescribed Keytruda, standard for advanced skin cancer elsewhere. Her provider refused coverage. Like countless Latin Americans, she sued to compel it.

Though drug pricing lacks openness, our probe showed Keytruda costs fluctuate sharply by nation, region, and system. Annual therapy spans $29,000 in Indonesia, $130,000 in Colombia, $208,000 in the U.S. In income-modest Guatemala, each three-week infusion nears $11,000 yearly totaling about $180,000.

"Who could cover that?" Chuc Xum asked, gesturing openly before replying. "Nobody."

Our work started remotely. We submitted requests, combed public files, crunched worldwide stats, and unearthed rulings to map Keytruda's true expense and corporate tactics sustaining its premium and monopoly. After spotting hundreds of Sam Colop-like Latin American suits, we saw high prices force insurers and facilities - strained by budgets and weak pharma leverage - to deny it absent judicial mandates. Files posed a key query: How does it feel when a drug exists yet stays beyond the needy?

I headed to Guatemala for answers, where Keytruda denial for insured and uninsured alike stood out starkly.

Upon finding her case, her kin mourned just a month post-passing. Colleague Jody García contacted them; we awaited their readiness. That Saturday at Sam Colop's, they convened for our chat as in old weekend family dinners. Privately, youngest Violeta whispered the house felt vacant without her mother since.

Post-victory, Sam Colop got Keytruda two years, though shortages caused gaps. Consistent or earlier dosing might have saved her, kin pondered. Our findings confirmed location often governs pricey, vital drug reach.

Marina drew solace from her mother's death preparations, including poetic directives. Violeta admitted lingering fury, needing time to process. Chuc Xum listed lost routines - home sightings, talks, spontaneous walks or dinners - culminating in his deepest wish: aging alongside his beloved.

Playing God

In January, Jody García phoned with reporter-thrilling news: a public records reply packed revelations.

We had sought, via info campaign, Guatemala's health ministry data on subsidized Keytruda patients publicly. A stark letter from oncologist Julio Ramírez, Quetzaltenango public hospital oncology chief - one of three nationwide - detailed it.

Only one got Keytruda, Ramírez noted, "not the sole case but due to supply over nearly two years." Two more approvals awaited; he must choose recipients.

Document-wise, mere stats: three added. Yet it spotlighted scarcity molding choices by the lead oncologist in Guatemala's second city.

As global records access erodes, such gems feel triumphant. Still, Ramírez's note skipped: How pick amid shortage?

Globally, Keytruda debates hit pricing, patents, billions. It starred in policies, earnings, breakthrough headlines. But at this hospital's unit, stakes were visceral.

Dawn Friday in early February, we met 50-year-old Ramírez in a tight office near a packed ward, patients in blue plastic hallway seats. Superhero scrubs, black hair with white forelock. His tone lit up recounting the 16-year-old unit handling 40 weekly cases.

Keytruda topped many unmet needs, he said, clutching a med list he requests despite budget shortfalls. He accepted limits, especially pricier ones. "Super costly - no system endures it," he noted. "I couldn't afford it. I'd sue too if needed."

Free access for three marked progress, he stressed.

"But selection criteria?" I pressed.

"What choice? Play God," he replied, voice edged.

His candor stunned. I anticipated biomarker-efficacy-cost talk. Instead, raw triage: first suitable arriver by diagnosis-tests got it. "That's it."

Miles to Survive

That February, we met Ramírez's initial Keytruda patient at the hospital. In 2022, 65-year-old Alberto Xum learned of metastatic kidney cancer lung-spread. In 2024, first approval made him priority; infusions started February. By our visit, two-year course neared end.

Triweekly, he caught 3 a.m. buses for Quetzaltenango tests-labs, overnighted nearby, infused next morn, then homeward.

Samayac return: nominally 2.5 hours, stretched by roads, jams, transit lags. Ours took four - one stalled at bridge fixes.

We found Xum, kin outside their church's huge weekend gathering in town center. Uninsured artisan crafting leather hats-souvenirs, he warmed fast, story-ready. In 2024, prior med's $2,000 triquarterly toll nearly quit him. Ramírez's free Keytruda news floored him.

"Doctors don't solo-prescribe that - immense worth," he said, crediting heaven. "Not mortal. Divine."

Keytruda shrank Xum's tumors sans remission. Rashes, diarrhea sideline him bus-ward on sales trips to festivals, awaiting next ride. Yet controlled cancer gratifies him.
