Republican Sen. Mitch McConnell has spent nearly a month in medical care, with scant updates on his condition, fueling speculation about his status and insights shared with his closest advisors, such as his spouse Elaine Chao.

McConnell and Chao connected via a shared acquaintance before exchanging vows in a discreet Capitol event in 1993, forming a notable Republican duo.

McConnell has represented Kentucky in the Senate for more than four decades. Chao broke barriers as the initial Asian American woman in a presidential cabinet following her 2001 confirmation as labor secretary under George W. Bush. During Donald Trump's initial presidency, she took on the role of transportation secretary.

McConnell's Senate staff did not promptly reply to inquiries from Business Insider.

He was raised in Alabama and Kentucky before entering the Senate for Kentucky in 1985. He has held positions as both minority and majority leader.

He is completing his current term without plans to run again in 2026.

Elaine Chao, 73, held the post of US transportation secretary in President Trump's opening term.

Chao became the first Asian-American woman named to a presidential cabinet. She earlier served as labor secretary from 2001 to 2009 under George W. Bush.

Chao arrived in the US from Taiwan at age eight and was raised in New York.

McConnell and Chao were introduced by mutual friend Stuart Bloch in Washington, DC during the early 1990s.

Bloch and McConnell first connected in the 1960s, according to The New York Times. Bloch was wed to Julia Chang Bloch, a mentor to Chao.

Bloch informed The New York Times that he hosted McConnell for a candlelit meal with Chao.

"I won't claim sparks flew," Bloch told the publication, "as that does not fit their style."

Bloch also called Chao a "tiger wife" composed of strong resolve.

They wed on February 6, 1993, during a private service in the US Capitol chapel.

They selected the date for three reasons, The Washington Post noted: it marks Ronald Reagan's birthday, it preceded Chao's 40th birthday, and it aligned with a Senate recess week.

McConnell had an earlier marriage that produced three children.

McConnell's initial spouse, Sherrill Redmon, later pursued work as a feminist scholar. They parted ways in 1980.

In 1998, the pair described their household task division to The Washington Post.

McConnell handled cooking occasionally and managed laundry, whereas Chao took charge of home repairs, The Washington Post indicated.

Chao frequently joined her husband on the campaign trail and earned the label "campaign hugger," as Time magazine noted.

McConnell experienced polio in childhood and finds backslapping uncomfortable, leading Chao to embrace supporters on his behalf.

"She listens intently and recalls specifics about individuals," Kelly Westwood, leader of the Kenton County women's Republican group, shared with Time in 2014. "She reconnects after months and inquires about a past arm injury or a city council bid. She retains every detail."

Chao has appeared in campaign spots supporting McConnell's stance on women's topics. A 2014 ad highlighted his role as co-sponsor of the 1991 Violence Against Women Act, though Talking Points Memo noted his opposition to the final 1993 measure and later renewals in 2012 and 2013.

In 2017, following Trump's criticism of McConnell on X, Chao told reporters she stands by her man - both of them.

After McConnell remarked on "excessive expectations" from the White House during a Kentucky event in August 2017, Trump posted on X, then Twitter, in reply.

"Senator Mitch McConnell said I had 'excessive expectations,' but I don't think so. After 7 years of hearing Repeal and Replace, why not done?" Trump wrote.

Trump followed up the next day: "Mitch, get back to work and put Repeal and Replace, Tax Reform and Cuts and a great Infrastructure Bill on my desk for signing. You can do it!"

When asked about Trump's remarks after an infrastructure session at Trump Tower, Chao answered: "I stand by my man - both of them."

The Washington Post noted McConnell's wealth grew seven times over a decade, rising from $3.1 million in 2004 to $22.8 million in 2014.

The Washington Post linked the rise in McConnell's net worth to an inheritance received after Chao's mother passed in 2007.

Chao's father, James Chao, established the shipping firm Foremost Group in New York in 1964.

In November 2022, McConnell opposed safeguards for interracial and same-sex marriages under the Respect for Marriage Act.

The Respect for Marriage Act established federal recognition for same-sex and interracial unions, obligating states to honor valid marriages from other states. McConnell joined 36 Republicans in voting no, while 12 GOP senators and all Democrats backed it.

McConnell offered no public comment on his vote.

In March 2023, McConnell entered the hospital with a concussion and fractured rib following a fall at the Waldorf Astoria in Washington, DC.

"This evening, Leader McConnell tripped at a local hotel during a private dinner. He has been admitted to the hospital where he is receiving treatment," a McConnell spokesperson stated to Business Insider in 2023.

McConnell had fractured his shoulder in 2019 after a fall at his Kentucky home.

Chao issued no public remarks on his condition during those events.

McConnell paused mid-speech and seemed disoriented during press events in July and August 2023.

The Capitol physician attributed the incidents to the March 2023 concussion and approved his continued service, noting in a statement that "occasional lightheadedness is not uncommon in concussion recovery and can also be expected as a result of dehydration."

On June 14, McConnell was admitted to the hospital following a dispatch of emergency responders to his location.

He has stayed in care for nearly four weeks.

McConnell's office issued statements noting the senator "appreciates the outpouring of support he's receiving while he continues his recovery in the hospital" and is "receiving excellent care," yet provided no specifics on his ailment.

Dispatcher logs showed responders performed CPR on an unconscious person, unnamed, The New York Times reported.

Republican Sens. John Thune and John Barrasso indicated they had spoken with McConnell by phone, with Barrasso describing him on Tuesday as "fully engaged" and "eager to get back to the Senate."

Chao was visiting China at the time of McConnell's hospitalization and did not return right away.

Chao was in China on June 12 - two days prior to McConnell's admission - and met Chinese Vice President Han Zheng on June 17 in Beijing during what a spokesperson described to The Louisville Courier Journal as "a long-planned trip in China to support her family's philanthropic endeavors."
